Terms of Use
By completing the Enquiry Form & Health Summary you are sharing information with JB Massage Therapy & Wellbeing. This will not be used as any form of medical diagnosis. The information will determine a course of action for therapeutic massage most suitable to your needs.
On submission of the form you will receive an acknowledgement via email, and/or follow-up telephone call.
This is not a binding contract, and any payment for services will be made directly at the time of treatment, either immediately before or after. Appointments will be confirmed with Jude Beckett directly, or an agent working on her behalf eg. Wakefield Osteopaths reception.
